Regulations of the Długosz & Długosz Dentistry Clinic
I. General Provisions
- These regulations define the rules for the provision of health services by NZOZ Długosz & Długosz Stomatologia, located in Kraków, at ul. Jana Zamoyskiego 73/U3, 30-519, NIP: 679-217-01-97, REGON: 351210079.
- The medical entity operates under the name: NZOZ Długosz & Długosz Stomatologia and is registered in the register of medical entities (RPWDL).
- The RPWDL is maintained by the Governor of Małopolska.
- The medical entity may also be referred to as the clinic or dental office.
- The clinic provides health services exclusively on a private basis in accordance with applicable laws, particularly the Act of April 15, 2011, on medical activity (Journal of Laws of 2022, item 633, as amended).
- The regulations are available for inspection at the clinic’s reception desk and on the website www.dlugosz.pl.
II. Scope of Services
- The clinic offers dental services in the following areas:
a. prevention,
b. diagnostics,
c. general dentistry,
d. dental surgery,
e. implantology,
f. prosthetics,
g. orthodontics. - A detailed list of services and a price list are available at the reception desk and on the clinic’s website www.dlugosz.pl.
III. Appointment Scheduling Rules
- Appointments can be scheduled in person, by phone at 12 259 81 80, 601-480-198, or 604-920-404, or via the online registration system at:
https://app.felgdent.com/extcal/004335049634870. - Patients must provide accurate personal and current contact information.
- Cancellation or rescheduling of an appointment should be made no later than 24 hours before the scheduled time. Otherwise, the clinic reserves the right to charge a fee for the missed appointment in accordance with the price list.
- The clinic requires a prepayment for appointments, which must be made at the time of registration. Prepayments are made via electronic banking to the following account: MBank 64 1140 2004 0000 3902 7842 7570, with the patient’s full name in the payment title.
- The prepayment amount is PLN 200.
- Prepayments are mandatory for:
a. first-time patients,
b. patients who have missed an appointment without providing at least 24 hours’ notice. - If an appointment is canceled within the period specified in point 3, the prepayment will be refunded or transferred to another date.
IV. Service Provision Rules
- Services are provided after obtaining the patient’s consent to the proposed treatment.
- Before treatment begins, the patient is informed in detail about the planned procedure, including expected costs and potential risks.
- The clinic is committed to adhering to professional ethics and ensuring the highest quality of services provided.
V. Rights and Responsibilities of Patients
- Patients have the right to:
a. receive detailed information about their health condition and proposed treatment methods,
b. have their dignity and privacy respected during the provision of health services,
c. submit comments and complaints regarding the services provided. - Patients are obligated to:
a. adhere to hygiene standards and follow the dentist’s recommendations,
b. pay for services on time,
c. inform the dentist of any significant changes in their health condition.
VI. Payments
- Payments for services can be made in cash, by card, or via bank transfer in accordance with the price list valid on the day the service is provided.
- The clinic reserves the right to request an advance payment for planned procedures.
VII. Complaints and Grievances
- Patients may submit complaints and grievances via email to stomatologia@dlugosz.pl or in writing to NZOZ Długosz & Długosz Stomatologia, ul. Zamoyskiego 73/U3, 30-519 Kraków.
- Complaints will be reviewed within 14 business days from the date of receipt.
VIII. Medical Records
- Medical records are made available to patients or persons authorized in writing by the patients, in accordance with the provisions of the Act on Patients’ Rights and the Patient Ombudsman.
- Medical records may also be provided to authorized entities, such as:
a. healthcare providers when necessary for the continuity of care,
b. public authorities, professional self-governing bodies, and national and regional consultants, as required for the performance of their duties, particularly in terms of supervision and control,
c. other entities authorized under separate regulations. - The clinic does not charge fees for providing access to medical records.
IX. Treatment of Minors
- When treating minors, appointments are conducted in the presence of a parent or legal guardian.
- Subsequent appointments may be conducted in the presence of a guardian authorized in writing by the parent or legal guardian. Authorization must include the names, ID series and numbers, and addresses of both the parent/legal guardian and the authorized guardian.
- For patients aged 16–18, consent is required from both the parent/legal guardian and the patient. If the parent/legal guardian cannot be present at subsequent appointments, a declaration of consent may be signed for treatment in the presence of third parties.
X. Telephone Consultations
- Dentists do not provide consultations over the phone.
- The clinic does not offer emergency services for pain or other dental issues.
XI. Financing of Services
- Health services at the clinic are fee-based and are not publicly funded.
- Patients can review the price of services before they begin. The dental price list is available at the clinic’s reception and on the website https://www.dlugosz.pl/cennik-uslug/.
- Payment for services is due upon completion.
- Payment can be made in cash or by card, with a fiscal receipt provided as confirmation.
- Invoices for health services are issued upon the patient’s request.
XII. Final Provisions
- The clinic reserves the right to amend these regulations.
- Patients will be informed of any changes in advance through announcements at the reception desk and on the website www.dlugosz.pl.
- These regulations take effect on February 1, 2025.
